Forms that must be printed can either faxed or mailed to the church office (see Contact Us).  -- If a form is an ONLINE form, you will be notified when we have processed your request.

Building Usage Form - for Meetings/Special Events - use this ONLINE FORM when scheduling your meetings or for any special event where you will be needing to use a room at the church or will be needing supplies such as paper goods, TVs, VCR/DVD players, etc.  OR you CLICK HERE to print this form at home and bring it to the church office

Nursery / Preschool Registration Form - print this form and bring the completed form to the nursery with your child to register your child for the 2008-09 year.

Permission Slip/Medical Release Form - this form is required for your child to participate in some Children's and Youth activities -- You may be requested to fill out another of these forms depending on the specific activity.

Vehicle Use Form - this form is required if you plan to use the church Bus or Van (for church activities only)
